✔️ Side Effects

Generally well-tolerated. Common side effects include:

Gastrointestinal: Constipation, gas, indigestion, abdominal pain

Other: Headache, infection, rash, muscle/joint pain, fatigue, back pain

✔️ Dosage & Administration

• Adults with high cholesterol or mixed lipid disorders: Start at 10 mg daily; may increase to 80 mg daily depending on response and LDL reduction needed.
• Children (10–18 years): Start at 10 mg daily; max 20 mg unless treating familial hypercholesterolemia, in which case max is 80 mg.

Coronary event prevention: Begin at 10 mg/day, adjust based on lipid profile.

Can be taken any time of day, with or without food. Lipid levels should be re-evaluated within 2–4 weeks of dose changes.

Continue cholesterol-lowering diet during treatment.

• Elderly: Slightly higher drug levels; typically more effective at lowering LDL.
• Children: Data limited; use under specialist supervision.
• Renal Impairment: No dose adjustment needed.
• Hemodialysis: Unlikely to clear the drug due to high protein binding.
• Liver Impairment: Significantly higher plasma levels; use with caution.

✔️ Drug Interactions

• Increased risk of muscle toxicity: When used with cyclosporine, fibrates, niacin, erythromycin, or azole antifungals.
• Antacids: Reduce atorvastatin plasma levels by ~35% but don’t affect LDL-C reduction.
• Colestipol: Lowers atorvastatin levels but enhances LDL-lowering effect when combined.
• Digoxin: Levels rise ~20%; monitor.
• Oral contraceptives: Increases hormone levels; consider when choosing a contraceptive.
• Warfarin: No significant impact on clotting time.

✔️ Contraindications

• Allergy to atorvastatin or any excipients.
• Active liver disease or persistent unexplained liver enzyme elevation.
• History of adverse effects from statins.

✔️ Pregnancy & Lactation

• Pregnancy: Not recommended—may harm the fetus. Discontinue if pregnancy is confirmed or planned.
• Lactation: Atorvastatin may pass into breast milk and could harm a nursing infant; avoid use while breastfeeding.
